Haley Nycole Keely, 4, of Kingman, Ariz., died at Phoenix Children's Hospital Nov. 14, 2004 after a nine-month battle with cancer.
She was born Oct. 2, 2000 in Newberg, Ore.
She is survived by her parents, Gabe and Kim Keely and sister, Erica Shea, of Kingman, Ariz.; grandparents, John and Velvet Novak of Payson, and Ron and Marti McDaniel of St. Johns, Ariz.; and Aunt Sami Novak of Payson.
Funeral services will be held at 11 a.m. Nov. 19, 2004 at Church of Jesus Christ of Latter-day Saints Stake Center in Kingman.
